In this paper, the pathogenetic and clinical significance of antigen - specific B - cell activation in early rheumatoid arthritis (RA) is considered. It is assumed that the cause of immunopathological reactions is a violation of the regulation of the immune response, resulting from an imbalance in the function of T and B lymphocytes, namely the immunodeficiency of the T-lymphocyte system, which leads to uncontrolled synthesis of B-lymphocytes of antibodies, in particular IgG.
The existence of collagen sensitive B cells in early RA suggests that in addition to the presence of appropriate antibodies, the ability of lymphocyte cultures to spontaneously produce antibodies to collagen II in early RA [1] and collagen induced arthritis, an increase in the proliferative response to cartilage proteoglycans. In the study of supernatant of short-term cultures of peripheral lymphocytes, the presence in the population of autoantibody-forming cells with selective metabolic activity in the connective tissue structures of the synovial membrane in early RA [1] was shown, and the proliferation of B cells in the presence of synovial cells in patients with early RA was enhanced.
