The aim of this review is to investigate the role of laparoscopy and pneumoperetonium in the formation of postoperative adhesions in the abdominal cavity. Despite the proposed approaches to reducing postoperative abdominal adhesions, the main clinical problem remains intestinal obstruction, chronic pelvic pain, female infertility and technical difficulties during repeated operations. Adhesions develop after 50-97% of abdominal and 60-90% of gynecological operations, due to incomplete lysis of cellular exudates by fibrin that occurs when the peritoneum is damaged. Although human studies are lacking, animal data indicate that the incidence of peritoneal adhesions can be reduced by more than 80% with adequate conditioning of the pneumoperitoneum. The data presented in this review may open up new possibilities for adhesion prevention in laparoscopic surgery.
