In the last few decades, ideas about seronegative spondyloarthritis (SSPA) have changed significantly and new diagnostic ones have appeared (ASAS, 2009 and 2012) based on two main diagnostic components: the presence of a genetic marker – HLA B27 and sacroiliitis confirmed by MRI or radiography, but as practice shows, the presence of these criteria did not help clinicians in the early diagnosis of SSpA in the endemic brucellosis zone. It is known that with SSpA, the development of bilateral sacroiliitis is more characteristic, but little is known that the same processes can occur in the ileosacral joint and in other diseases, such as chronic brucellosis, which is difficult to differentiate due to the polymorphism of clinical manifestations and the lack of information of specific serological analyses. Early diagnosis and detection of brucellosis in people whose profession is not related to animal husbandry remains a problem. At the same time, a timely diagnosis, as well as adequate therapy, will help to avoid the chronization of the process and the development of long-term disability and disability.
