The article presents an up–to-date review of the literature devoted to one of the important problems of biliary surgery - the diagnosis and treatment of "fresh" trauma of the main bile ducts, especially in combination with vascular damage of the hepatoduodenal ligament. The injury of the LC is the most dangerous and severe complication of HE, often patients undergo numerous surgical interventions and become disabled. Iatrogenic damage to the LVM is characterized by high rates of complications, mortality and unsatisfactory long-term results. In 30% of cases, an injury to the LC leads to a fatal outcome, and the average life span of patients after reconstructive operations on the LC for their injury is about 7 years. To choose the most optimal way to treat such an injury, most authors distinguish small and large trauma of the LC. If the choice of surgery for a minor injury does not cause any controversy, then the treatment of patients with complete intersection or excision of the main bile duct continues to be discussed on the pages of scientific literature. For major duct injury, both restorative and reconstructive interventions are proposed. However, most authors note the obvious advantages of reconstructive interventions, since reconstructive operations at the same time give unsatisfactory results. The operation of choice is recognized as the formation of a high hepaticoejunoanastomosis according to Ru. The expediency of drainage, especially frame drainage, during reconstructive and reconstructive operations on the bile ducts remains relevant. The priority is the need to perform operations in one stage in order to save patients from repeated surgical interventions.
