THE EFFECT OF SURGICAL ACCESS ON THE RESULTS OF OPEN RECONSTRUCTIONS OF THE AORTO-FEMORAL SEGMENT IN PATIENTS WITH DIFFERENT PARAMETERS OF THE CONSTITUTION

Relevance: The high frequency of postoperative complications and mortality remains the main problem of open interventions on the aorto-femoral segment. Objective: To study the relationship between the constitutional parameters of the patient and the surgical access used in open aorto- bifemoral reconstructions. Materials and methods: 220 patients underwent surgery for lesions of the aorto-femoral segment of type C and D according to TASC II, using laparotomy access (group I - 117 patients), and retroperitoneal access (group II - 103 patients). The groups studied the incidence of systemic postoperative complications and mortality, as well as the parameters of the constitution, assessing the presence or absence of overweight and obesity. The χ2* criterion and the Mann-Whitney U-test** in the Statistica 10 package were used to calculate the data. Results: In group I, 31 (26.4%) cases of systemic complications were noted, among which 21 (67.7%) patients were overweight or obese. Deaths in group I were recorded in 14 (11.9%) clinical cases, of which 11 (78.6%) patients are obese or overweight. In group II, the number of systemic complications was 28 (27.2%) cases, among which there were 10 (35.7%) overweight patients. Mortality in group II was recorded in 11 (10.7%) cases, of which 2 (18.2%) patients had signs of overweight or obesity. Conclusions: In patients with signs of obesity, the use of retroperitoneal access during open reconstructions of the aorto-femoral segment is associated with the lowest number of postoperative systemic complications and mortality, in contrast to laparotomic access.
